How You Will Fulfill Your Potential Responsibilities ESSENTIAL FUNCTIONS Interprets, reviews and evaluates medication orders for optimal dose, dosage form, frequency, concentration, duration, drug-drug interactions, drug-disease state interaction, drug allergy interaction, and potential cross allergenicity, taking into account the patient’s special or age related needs, to ensure optimal drug therapy, high quality patient care and timely dispensing of accurate and appropriate medications. Dispenses medications by reviewing patient specific bulk medications and pyxis fills and refills for accuracy of the right medication, dose, dose form, strength, size, quantity to ensure accurate, timely and safe pharmaceutical care. Compounding patient specific and batch compounded sterile preparations (CSPs) by ensuring the right drug, route, dosage form, strength size and quantity to deliver the prescribed dose. The appropriate dose and final concentration of active ingredients, measurement of prescribed dose, appropriate diluents and volumes and admixing such CSPs is performed by the pharmacists to ensure safe high quality patient care and timely dispensing of CSPs in accordance with USP 797 standards. Provide comprehensive clinical information about medications and patient-specific drug information to nurses, physicians and other health care providers and professionals and patients to ensure optimal pharmaceutical care. Consult with other healthcare professionals concerning clinical guidelines and therapeutic substitution of pharmaceuticals fostering optimal use, safety and cost effectiveness of drug therapy. Participates in process improvement teams within the department and organization to build strong relationships with staff members, demonstrates GFH core values and enhances financial viability
