Hartford HealthCare’s Pharmacy System has grown to include 9 inpatient pharmacies, 9 cancer center infusion pharmacies, 3 retail pharmacies, and our Ambulatory Care pharmacy department supporting more than 50 medical office practices. Collaboration between our numerous pharmacy services and other members of the health care team is key to our commitment to continually improving how we provide patient care. We work hard to support our pharmacists in their efforts to optimize safe, effective, and efficient pharmaceutical care for all the people we serve. We provide our pharmacists with the tools they need to be at their best, including access to advanced resources and state-of-the-art pharmacy software. Pharmacists can advance their careers both within their positions via our career ladder as well as movement into specialized and leadership roles. The Pharmacy Department at St. Vincent’s Medical Center operates using a decentralized pharmacy model, meaning pharmacists are directly embedded within patient care units and medical teams rather than working solely from a central pharmacy. This structure allows pharmacists to participate in daily interdisciplinary rounds, collaborate closely with physicians and nurses, and provide real time expertise on optimal drug therapy, improving both safety and clinical outcomes. St. Vincent’s also proudly supports an American Society of Health System Pharmacists (ASHP)–accredited Pharmacy Residency Program, which offers postgraduate clinical training for new pharmacists. Through this program, residents gain hands-on experience across multiple specialties, participate in research and quality improvement initiatives, and develop the advanced skills necessary for contemporary hospital pharmacy practice. Across all areas of practice, pharmacists at St. Vincent’s Medical Center play a critical role in the medication use process. They ensure the appropriate selection, dosing, administration, and dispensing of all medications, while monitoring safety, effectiveness, and potential drug interactions. Their involvement promotes evidence based therapy, reduces medication related errors, and supports high quality patient care throughout the medical center.
