Requirements

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy required
  • Current, unrestricted Ohio pharmacist license required.

Nice To Haves

  • advanced degree or management certification preferred.
  • Experience in a Federally Qualified Health Center (FQHC), hospital, or multi-site healthcare organization preferred.

Responsibilities

  • Provide safe, accurate, and timely dispensing of medications in accordance with federal, state, and organizational policies.
  • Review and assess medication orders to identify, prevent, and resolve potential drug-related problems.
  • Conduct drug utilization reviews and evaluate drug interactions, allergies, and contraindications.
  • Provide clear and compassionate medication counseling and education to patients and staff.
  • Participate in interdisciplinary care team meetings and collaborate with providers to support integrated patient care.
  • Support pharmacy operations, including inventory management, documentation, and compliance activities, including 340B requirements as assigned.
  • Serve as a resource for pharmacy-related questions from patients and clinical staff.
  • Assist with quality assurance activities, audits, and reporting as directed.
  • Collaborate effectively with physicians, nursing staff, pharmacy leadership, and external partners to ensure high-quality patient care.
