About The Position

The Pharmacist at the Long Island State Veterans Home is responsible for providing comprehensive pharmaceutical care to residents, ensuring optimal medication therapy through regular reviews and consultations with the clinical team. This role involves dispensing medications, preparing sterile products, and advising medical staff on medication availability and rational use.

Requirements

  • B.S. Pharmacy
  • Long term care or institutional pharmacy experience.

Responsibilities

  • Enter medication orders onto computerized medication profiles and update as required, checking for drug interactions and optimum drug therapy.
  • Dispense medication to residents as prescribed.
  • Perform monthly drug regimen reviews and report findings to the Director of Pharmacy.
  • Prepare sterile products in accordance with acceptable standards of practice and departmental procedures.
  • Coordinate the prompt and timely distribution of all medication and related information.
  • Dispense controlled substances in accordance with laws and departmental policies and procedures.
  • Consult and advise medical staff on the availability and rational of medications ordered.
  • Order pharmaceuticals through wholesalers daily to maintain sufficient inventory levels.
