This position is to support home infusion pharmacy in Peoria. Home Infusion Pharmacy specializes in providing home intravenous therapies, clinical therapy management and other related support services not available from most traditional retail pharmacies. We serve patients throughout Illinois. Schedule is for 40 hours week (Week day 8 hour shift between 6:30am-6:00pm, weekend on call rotation every 7th to 9th weekend). The Clinical Pharmacist provides clinical pharmacy management for home infusion therapy patients. Performs initial and on-going patient assessments. Interprets prescription and physician orders and performs computer entry. Responsible for all aspects of intravenous admixture and product distribution functions, by either performing personally or by appropriate direction and supervision of pharmacy technicians. Discusses patient care as needed with others involved in providing service, including physicians and home health nurses. Identifies potential adverse drug events, including risks for medication errors. Reviews reported adverse events, and identifies opportunities for improvement. Rotates 24/7 on call coverage with other pharmacists. Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. This requires that the individual demonstrate knowledge of the principles of growth and development.

  • Licenses/Certifications: PHRM-Pharmacist.
  • Knowledgeable of state, federal and JCAHO regulations.
  • Analytic skills necessary to organize and oversee the work of others and evaluate quality and outcome of care provided.
  • Ability to develop and implement policies and procedures.
  • Must provide own transportation and auto insurance.
  • Maintain a valid driver’s license.
  • Demonstrate ability to work independently and execute own workload.
  • Ability to use computer workstation and operate infusion pumps, clean room pharmacy equipment, and office equipment.
  • Interpersonal skills for effective verbal and written communication.
  • Responsible for taking on-call on a rotating basis.
  • Education: Doctorate of Pharmacy (PharmD.)
