The Clinical Pharmacist – SRT contributes to AHF by being assigned to more than one location, based on scheduling/geographical needs. Expanding primary functions of pharmacist such as providing individualized health promotion & disease prevention, including vaccine, long-acting HIV medicine administration, Diabetes monitoring, Nicotine reduction programs and other subject matters, in collaboration with other AHF Providers. The Clinical Pharmacist – SRT enhances the reputation of AHF by improving the outcomes of AHF patients, enhancing current AHF services, expanding opportunities for professional growth and satisfaction, and assisting with ongoing development of the AHF Healthcare System. You will be a passionate advocate for our top initiatives. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Clinical Pharmacist – SRT will work as a part of a close-knit team to provide individualized health promotion and disease prevention, including the administration of immunization, Diabetes and Smoking Cessation programs, as well as other subject matter expertise (SME) programs developed by AHF, with appropriate collaborative drug therapy management authority. Includes prescriptive authority where this is legally & organizationally authorized, such as PrEP and PEP therapies. Schedule: Varies by site, usually 8 hour shifts Monday-Friday Main Two Sites in Baton Rouge, LA 8:30am-5pm, 8am-4:30pm Travel required to: New Orleans, Missisissippi , South Carolina
