Requirements

  • Must have one of the following by the start of the residency program/hire date: o Doctor of Pharmacy (PharmD) from an Accreditation Council for Pharmacy Education (ACPE) accredited degree program (or one in process of pursuing accreditation) required or o Foreign Pharmacy Graduate Equivalency Committee (FPGEC) certificate from the National Association of Boards of Pharmacy (NABP). At a minimum, the program must be a five-year pharmacy degree program required
  • INTERNRX - State of California Intern Pharmacist License obtained prior to July 1 required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required within 90 days of hire or transfer into position
  • ACLS - Advanced Cardiac Life Support Certification required within 90 days of hire or transfer into position
  • RPH - Registered Pharmacist with valid State of California Pharmacist License required within 120 days of hire or transfer into position
  • Participation and adherence to the rules of the ASHP Resident Matching Program is mandatory for all candidates of the CMC Residency Program.
  • All pharmacy residents must have a current and valid California intern license and have taken the North American Pharmacist Licensure Examination (NAPLEX) and California Practice Standards and Jurisprudence Examination for Pharmacists (CPJE) for California Pharmacist licensure within thirty (30) days of the start of the residency program.
  • It is solely the responsibility of the resident to comply with all licensing requirements.
  • If the resident is not California Pharmacist licensed within one hundred twenty (120) days of the start of the program, the Pharmacy Residency Program reserves the right to terminate the resident’s contract. This licensure is vital to the ability of the resident to fulfill the requirements of the residency program and is non-negotiable.

Responsibilities

  • Provides consultation services including assessment of patients, performing appropriate calculations and providing recommendations on drug therapy and dose.
  • Verifies accuracy of medication compounded and dispensed from pharmacy services.
  • Provides drug information to medical, nursing and/or ancillary staff as needed.
  • Interprets physician orders and intervene when appropriate for clarification or correction.
  • Reviews the patient profile for allergies, interactions and appropriateness of prescribed medications before dispensing.
  • Checks dosage for appropriateness for pediatric, adult and geriatric patients.
  • Carefully checks the work of technicians before initialing labels. Directs technicians to perform tasks appropriate to current priorities in order to maintain work flow.
  • Participates in medication use evaluations, record pharmacist interventions and report adverse drug events.
  • Represents the organization and the department in a professional manner at all times.
  • Performs miscellaneous job related duties as assigned.
