About The Position

Adventist Health Glendale is proud to be part of the thriving, diverse community since 1905. Job Summary: Formulates and executes medication management systems and pharmacotherapy services. Ensures that medications, supplies, and drug information are furnished to the customer. Collaborates with dedicated team members across disciplines to create original policy and models to promote appropriate and effective medication utilization and harmonious application across all disciplines. Monitors drug therapies, identifies and troubleshoots potential medication safety hazards and documents all interactions for medical review. Assists in making recommendations on programs, process improvement or development of new pharmacy services.
