The Ambulatory Care/ Oncology/Hospital Pharmacist is responsible for all aspects of ambulatory care/oncology and hospital pharmacy. The pharmacist is responsible for dispensing prescription and non-prescription medications to patients in the inpatient, outpatient and emergency department settings. The Pharmacist provides patients with information about their medication regimen and helps ensure understanding of the medications. The Pharmacist must have scientific aptitude, excellent communication skills, and be detail oriented. The Pharmacist advises physicians and other health care practitioners on the selection, dosages, interactions, and potential side effects of medications. The Pharmacist will perform duties in the Disease and Medication Management clinic, Oncology and Outpatient Infusion and hospital pharmacy. While keeping current on regulatory compliance and tracking of quality assurance assessments to promote quality/continuity of care. The Pharmacists monitor the health and progress of patients in response to drug therapies. Pharmacists will also help patients manage chronic conditions such as anticoagulation, asthma, COPD, heart failure, hypertension, and medication management per collaborative practice agreement in conjunction with an interdisciplinary team.
