The position involves monitoring drug therapy for assigned patients, assessing therapeutic endpoints and potential adverse effects by evaluating relevant lab and diagnostic parameters. The role requires contacting prescribers with patient-specific recommendations to improve outcomes, decrease adverse effects, and lower costs. The pharmacist will accept prescriber changes in therapy and communicate these changes per procedure with the patient’s nurse. Additionally, the pharmacist will evaluate and intervene when appropriate after being alerted by surveillance software rules, which include renal and hepatic dosing, anticoagulation monitoring, and antibiotic stewardship. The role also includes providing pharmaceutical services for outpatient and inpatient pharmacies, reviewing and reconciling admission orders, allergy and medication histories, and clarifying any discrepancies with the prescriber, patient, and family. The pharmacist will actively participate in patient care rounds and treatment teams, focusing on interventions and education efforts to optimize patient drug therapies and improve medication prescribing patterns. Furthermore, the pharmacist will provide drug information services to staff and patient education as requested, identify performance improvement and quality assurance initiatives for the Pharmacy Department, and assure compliance with National Patient Safety Goals and Medication Management standards of The Joint Commission.
