About the position

The position involves various job functions related to patient care and medication management. The primary responsibilities include profiling and transcribing patient information, dispensing medications, checking for accuracy, preparing sterile IV medications, participating in clinical initiatives, providing drug information, and improving performance and customer satisfaction. Each function is critical to ensuring optimal patient care and safety in medication management.

Responsibilities

• Reviews patient profiles to assure appropriate therapy based on patient specific information.
• Transcribes all medication orders accurately and timely, verifying therapy, correct medication, route, dose, and frequency.
• Makes recommendations for alternate treatments when necessary.
• Records relevant information and drug waste in EHR.
• Dispenses medications, prescriptions, and miscellaneous items accurately and promptly.
• Processes requests via phone, window, fax, and computer in a professional manner.
• Obtains medications to provide optimal patient care considering patient admission status.
• Checks unit dose and medications for accuracy before placement in automated dispensing machines.
• Prepares and supervises aseptic compounding of IVs, including antineoplastic agents.
• Participates in clinical initiatives and initiates appropriate clinical interventions.
• Provides drug information to healthcare professionals and patients.
• Participates in performance improvement and cost savings initiatives.

Requirements

• Strong knowledge base of IV equipment and sterile technique.
• Ability to demonstrate compliance with Pharmacy and Therapeutics Committee initiatives.
• Experience in monitoring patient therapies and making recommendations.
• Knowledge of drug information resources and ability to research and interpret data.

Nice-to-haves

• Experience in clinical pharmacy settings.
• Familiarity with aseptic compounding and hazardous drug preparation.
• Previous involvement in performance improvement projects.
