About The Position

The Pharmacist position at UMass Memorial Health is a per diem role located in Southbridge, MA, requiring on-site presence. Requirements

• Doctor of Pharmacy (Pharm.D) degree; OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience.
• Licensure with the Massachusetts Board of Registration in Pharmacy.

Responsibilities

• Provides medication management services in a collaborative manner.
• Reviews patient medication history and educates patients on drug therapy.
• Establishes desired outcomes and develops/modifies therapeutic plans.
• Monitors patient outcomes and documents medication-related issues in electronic health records.
• Ensures timely and accurate dispensing of medications from the pharmacy area.
• Maintains and updates patient profiles with demographics, diagnosis, allergies, and current medications.
• Promotes the use of formulary drugs and coordinates procurement of non-formulary drugs.
• Detects, monitors, documents, and reports adverse drug reactions and medication errors.
• Complies with established departmental policies, procedures, and objectives.
