About The Position

KLTC Pharmacy is a growing long-term care pharmacy dedicated to improving the health and well-being of the residents we serve. We pride ourselves on delivering high-quality, patient-centered pharmacy services while fostering a collaborative and supportive team environment. We are seeking a Licensed Pharmacist to join our team. In this role, you will provide clinical pharmacy services to long-term care residents, ensuring safe, effective, and compliant medication therapy. This is an excellent opportunity for a pharmacist who's passionate about geriatrics and interdisciplinary collaboration.

Requirements

  • PharmD degree from an accredited program
  • Active Pharmacist License (required, from Michigan)
  • Active Controlle Substance License (required, from Michigan)
  • Long-term care pharmacy experience preferred
  • Strong understanding of pharmacy regulations and compliance
  • Clinical knowledge with a focus on geriatric populations

Responsibilities

  • Provide clinical pharmacy services tailored to long-term care residents
  • Perform medication reviews and therapeutic consultations
  • Collaborate with physicians, nurses, and healthcare teams
  • Ensure compliance with all state, federal, and regulatory requirements
  • Oversee dispensing accuracy and provide clinical oversight
  • Utilize electronic health records for documentation and communication
  • Support and direct pharmacy technicians and staff as needed
  • Deliver exceptional customer service to patients and caregivers
  • Participate in team meetings and quality improvement initiatives
