Requirements

• Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.
• Bachelor's or advanced degree in Pharmacy required.

Responsibilities

• Ensure accurate medication management, including selection, dosing, administration, and dispensing.
• Conduct thorough medication order reviews to proactively identify, prevent, and resolve potential or actual drug-related issues.
• Recommend cost-effective, formulary-approved alternatives when clinically appropriate.
• Prepare and compound medications for precise and timely patient delivery.
• Act as a reliable source for drug information and provide educational resources to both medical staff and patients.
• Maintain essential communication with physicians and other clinicians to ensure seamless patient care.
• Supervise technical support personnel involved in the preparation, inventory, and dispensing of medications and supplies.
• Contribute to academic programs, which may include serving as a preceptor for pharmacy students.
• Provide efficient and effective patient counseling as required.
• Be available to float to different areas to ensure comprehensive pharmacy coverage.
