Pharmacist - Inpatient / Outpatient Float This pharmacist position works in both the Inpatient and Community Pharmacies. Hours: Pharmacist splits time between Inpatient and Community Pharmacies based on scheduling needs. Inpatient Pharmacy Weekend Rotation, two 12 hour shifts every 6 weeks. Primary hour coverage is within Inpatient Pharmacy and secondary coverage in Community Pharmacy. 32 Hours/Week (0.8 FTE) General Responsibilities: Provide pharmacist coverage in both the Inpatient and Community Pharmacies Assist with maintenance of inventory Inspect medication areas for outdated medications, proper storage, proper administration records, and availability of reference materials. Monitor drug therapy regimens checking for interactions, allergies, unapproved abbreviations, and appropriateness of drug and dose, providing consultation and clarification to providers as necessary. Assists in directing pharmacy technicians and support personnel. Interpretation and accurate entry of medication orders and patient medication profiles. Dispense medications, IVs, Chemotherapy, and checks products prior to dispensing to automated dispensing cabinets.

Requirements

  • Graduation from an accredited college of pharmacy
  • License in state of Iowa as a registered pharmacist
  • Continuing education in good standing.
  • Interpret medication orders (verbal/electronic/written) and transcribe to patient medication profiles accurately.
  • Dispense medications accurately and timely.
  • Complete all competence assessment requirements.
  • Provide courteous and cooperative service to internal and external customers according to service excellence standards.
  • Document clinical interventions.
  • Complete daily/monthly medication area inspections.
  • Communicate with providers, insurance or financial department to help problem solve issues with the medication orders, formulary or finances.
  • Provide drug information when needed and clinical recommendations for medication orders
