About The Position

The Clinical Pharmacist is responsible for providing medication therapy management services in collaboration with other providers to assure that drug therapy is safe, effective, efficient, and cost-effective. Pharmacy services provided include, but are not limited to, patient care rounds, admission and discharge medication reconciliation, medication therapy monitoring, medication orders management, patient education, drug information, drug dosing, and various scholarly activities. The Pharmacist performs both clinical and distributive tasks as necessary. The Pharmacist will provide training and education to pharmacy residents, students through practical and didactic experiences. Incumbent acts independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines with support from the Lead Pharmacist and Pharmacy Director. The pharmacist ensures that patient care is maintained through compliance with the department's policies, rules and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies. We provide pharmacy services in the Hospital, Specialty/General Clinics, Nursing Home, and work collaboratively with our Outpatient and Ambulatory Pharmacy teams. The incumbent will be expected to handle hazardous drugs (chemotherapy) with appropriate primary and secondary engineering controls.

Requirements

  • Pharmacy degree from an accredited college of pharmacy.
  • Minimum one (1) year hospital/community pharmacy experience.
  • Licensed to practice in the State of Wisconsin.

Nice To Haves

  • ASHP accredited pharmacy (PGY-1) residency training preferred.
  • LTC consultation experience is preferred.
  • Working knowledge of clinic-based operations preferred.
  • Certification by Board of Pharmacy preferred.
