About The Position

The Clinical Pharmacist position involves optimizing patient medication therapy by providing comprehensive clinical pharmacy services under the direction of the Clinical Coordinator or Manager and the Pharmacy Director. The role requires collaboration with the healthcare team to achieve patient care goals.

Requirements

• B.S. Pharmacy or Pharm D, or Master of Pharmacy.
• Active registration as a licensed pharmacist in the state of Texas.
• Sterile Compounding Certification.
• ACLS or PALS certification.
• Previous hospital experience with clinical responsibilities.

• Excellent oral and written communication skills.

Responsibilities

• Optimize patient medication therapy by providing clinical pharmacy services.
• Collaborate with the healthcare team to accomplish patient care goals.
• Perform medication dosing consultations and home medication reconciliation.
• Provide educational presentations to pharmacy staff and other healthcare staff as requested.
