About The Position

To provide the professional, clinical and technical knowledge, skills and expertise required for the provision of comprehensive pharmaceutical care that supports the mission, values and philosophy of Pharmacy Services and Sharp HealthCare. Will also provide assistance to the Antimicrobial Stewardship Pharmacist (ASP pharmacist) as needed.

Requirements

• Education pursuant to licensure requirement
• 3 Years experience in a hospital pharmacy practice or 1 Year completion of PGY1 Pharmacy Residency
• California Pharmacist License - CA State Board of Pharmacy - REQUIRED
• ACLS Certification (Advanced Cardiac Life Support) - American Heart Association - REQUIRED
• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association - REQUIRED

• 2 Years completion of PGY-2 Pharmacy Residency

Responsibilities

• Verification and review of medication orders
• Monitors therapy for potential or actual drug interactions
• Compliance with inpatient drug formulary
• Provides counseling to patients on medication use
• Participates in admission and discharge reconciliation
• Ensures that drug regimen is consistent with all Sharp medication use guidelines
• Completes a daily profile review for each patient
• Participates in clinical intervention program
• Communicates with nurses and physicians about therapeutic issues
• Documents interventions and patient contact
• Medications are prepared and dispensed accurately
• Assures controlled substances are dispensed with accuracy
• Demonstrates proficiency with automation and technology
• Participates in cost management initiatives
• Knowledge of and active participation in medication safety event monitoring programs
• Ensures security of medication storage and compliance with standards
• Fosters positive relationships through consistent, respectful, professional interactions with customers
• Orients and trains new pharmacy staff
