This position provides services at the TCRHCC main campus. Pharmacist is responsible for providing a comprehensive pharmacy and medication management service to patients and ensures a consistent level of service in accordance to TCRHCC policies. Direct patient care and services are provided to neo-natal, pediatric, adolescent, adult, geriatric, oncology, and obstetric patients. This position requires working rotational shifts: day, evening, nights, weekends, and Holidays. The position provides the opportunity for career advancement through tier levels of Pharmacist to Advanced Practice Pharmacist as determined by pharmacy experience. Advanced Practice Pharmacist serves as a clinical pharmacist for direct clinical pharmacy services for inpatient and outpatient departments. Responsibilities also include the administration of policies and procedures and directing programs and activities in accordance to TCRHCC's mission, vision, and values. Incumbent serves as the Clinic Manager of Operations at the designated clinic(s) and provides technical and administrative supervision to professional and non-professional employees, i.e. Advanced Practice I, Staff Pharmacists, Pharmacy Technicians, Pharmacy Assistants and Pharmacy Billers.

  • Conducts authorized outpatient pharmacy clinics for examination, diagnosis, and treatment of specialized clinic patients.
  • Obtains relevant health history, follows, and adjusts the developed treatment plan, and monitors the patient's outcome to the prescribed treatment plan.
  • Reviews inpatient and outpatient medical records for safety, appropriateness, and effectiveness of drug therapy with regard patient-specific factors.
  • Consults with Medical Staff when concerns arise and collaborates with nursing staff to evaluate medication use.
  • Reviews, assesses and inputs chemotherapy treatment plans for oncology patients, verifies medication orders to ensure accuracy of dosage, treatment schedule, route of administration and safe administration based on laboratory information and evidence-based practices.
  • Counsels patients, their representatives, and/or patient family members on safe and effective use of prescribed medications.
  • Participates in pandemic and/or disaster drills that TCRHCC has elected to take part in and including responding to actual disaster events.
  • Develops hospital staff's pharmacological knowledge by participating in clinical programs, training pharmacy staff, students, interns, externs, residents, and health care professionals.
  • Maintains pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies for continuing education.
  • Provides pharmacological information by answering questions and requests of health care professionals.
  • Doctor of Pharmacy or a Bachelor of Pharmacy degree from a school recognized by the American Council of Pharmaceutical Education.
  • Must have a current valid and unrestricted license to practice pharmacy in a State or Territory of the United States, or the District of Columbia.
  • Must have and maintain a current valid certification of Basic Life Support (BLS) from the American Heart Association.
  • Pharmacist I: Not required for entry level incumbents.
  • Pharmacist II: A minimum of one (1) year experience as a Registered Pharmacist.
  • Advanced Practice Pharmacist I: A minimum of two (2) years' experience as a Registered Pharmacist and a minimum of one (1) year experience as a Clinical Pharmacist.
  • Advanced Practice Pharmacist II: A minimum of five (5) years' experience as a Registered Pharmacist and a minimum of two (2) years' experience as a Clinical Pharmacist.
  • Must become a credentialed member of the TCRHCC Pharmacy Staff.
  • Possession of high ethical standards and no history of complaints and no prior records of substance abuse.
  • Reliable and dependable; reports to work as scheduled without excessive absences.
  • Maintains a positive professional attitude and demonstrates good interpersonal communication skills.
  • Knowledge of legal dispensing practices, federal pharmacy regulations and therapeutic incompatibilities.
  • Knowledge of sterile compounding (and USP 800 regulations) and non-sterile compounding procedures.
  • Knowledge of drug-drug, drug-disease state, and drug-allergy incompatibilities.
  • Knowledge of pharmacology, therapeutics of common disease states and treatment modalities, pathophysiology, and pharmacokinetics.
