The Pharmacist is responsible for providing medication management, clinical support and consultative services in a complex pharmacy operation. This position partners with key stakeholders to drive patient and family education, medication safety, quality assurance, customer service, process improvement and regulatory compliance. This position may also participate in education of pharmacy students and residents.

  • Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
  • Provide clinical pharmacy services to all patients in assigned area using Standard Operating Procedures (SOP). This includes dispensing of medication, completing review for all patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.
  • Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives as appropriate.
  • Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.
  • Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences and actively participating in department clinical meetings and activities.
  • Participate in regulatory, quality assurance and process improvement activities as required.
  • Meets all regulatory requirements.
  • Uses available information technology and other resources to support day-to-day work operations
  • Clinical pharmacists practice in an integrated team-based model.
  • Clinical Pharmacists have accountability for supporting a high-functioning pharmacy team.
  • The Clinical Pharmacist will provide all pharmaceutical care within their assigned areas including clinical services, drug distribution, regulatory compliance (TJC, CMS, etc.), drug information, medication safety, process improvement, financial and formulary initiatives, medication use evaluations, and education/development of staff, students, and residents, and other duties as assigned by a pharmacy leader.
  • The Clinical Pharmacist will develop and maintain intra and interdepartmental relationships, including working closely with nurses and physicians to achieve these goals.
  • This pharmacist will participate in all code situations in the emergency department including but not limited to Code Blue, Code Stroke, VTVF activations and Rapid Sequence Intubations.
  • Pharm. D. or bachelor's in pharmacy
  • Completion of a PGY1 residency or equivalent hospital clinical practice experience
  • Eligible for MN Pharmacy License
  • Completion of a PGY2 residency in Emergency Department pharmacy, Critical Care pharmacy (or other related field) or equivalent advanced hospital clinical practice experience in emergency department pharmacy.
  • BPS certification
  • Meets ASHP residency preceptor criteria
  • BPS certification
  • Experience precepting students, PGY1 and PGY2 residents
