Is able to serve as a resource in the provision of pharmaceutical care to patients in the emergency department.
Requirements
- Bachelor of Science in Pharmacy required; Pharm.D. preferred.
- Completion of a 1-year emergency pharmacy practice residency, specialty residency, emergency/trauma certification, or at least 3 years of pharmacy experience is strongly preferred.
- Current licensed pharmacist in South Carolina.
- ACLS, BLS, PALS certification required for ED/FED.
Responsibilities
- Medication Reconciliation is the primary role of the ED pharmacist. In collaboration with the nursing staff perform routine review and correction of home medication lists for patients admitted into the hospital via the ED, and for behavioral health patients being held in the FLEX area.
- Respond to all code STEMI patients while on duty to provide any assistance and immediate verification of orders or putting in the orders at the bedside for the provider. Review for recommendation for aspirin on arrival.
- Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternative to medical staff as needed.
- Be on stand by for procedural sedations in order to provide medication recommendations (such as if there are shortages) and help to dose medications. Draw up the sedation medications and enter the order in Cerner with correct times and dosages.
- Interprets and verifies medication orders in a timely manner. Maintains accurate, complete pharmacy patient medication profiles.
- Knowledgeable of and complies with laws, rules and regulations that govern the practice of pharmacy.
