This pharmacist will practice within a progressive clinical pharmacy model at Upstate Community Hospital. Applicants will join a talented team of pharmacy personnel to deliver exceptional pharmaceutical care. Pharmacists at Upstate participate in all aspects of care, including clinical patient monitoring, sterile product preparation/oversight, medication distribution, transitions of care management, and patient counseling/education. As an academic medical center, opportunities for developing pharmacy educational programs and participation in research projects exist and are encouraged. Pharmacists at Upstate have the opportunity to practice at the top of their pharmacy licensure.

  • Interpret medication orders and prescriptions to ensure they are complete, safe, rational, and compatible with institutional policy.
  • Prepare compounds and dispense medications for inpatient and ambulatory care use, including injectable sterile products and non-sterile compounds.
  • Perform duties in various practice locations, including inpatient, emergency departments, ambulatory care clinics, and transitions of care settings.
  • Provide information to health care practitioners to promote the safe and rational use of medications.
  • Provide drug monitoring services and maintain pharmacy records according to law and institutional policy.
  • Interpret policy and procedures for hospital staff.
  • Participate in teaching programs for the department and hospital staff.
  • Participate in the department's quality assurance programs.
  • B.S. in Pharmacy or Pharm.D.
  • NYS license and registration - or eligible - required.
  • Completion of PGY1 Pharmacy residency or equivalent experience required.
  • Previous hospital experience preferred.
