Ambulatory Health Care Services

Primary functions are assessing, planning, delivering, coordinating, teaching and providing patient disease management (anticoagulation, hepatitis C, lipid, smoking cessation, vaccinations, etc.) care/treatment and department services. On occasions assumes charge responsibility. Serves as preceptor for new hires, students and resource person for other staff.

  • Assessing patient needs and planning appropriate care.
  • Delivering and coordinating patient disease management services.
  • Teaching and providing education on disease management.
  • Assuming charge responsibility as needed.
  • Serving as a preceptor for new hires and students.
  • Acting as a resource person for other staff.
  • Graduate of an ACPE accredited School of Pharmacy.
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy required.
  • Licensed Registered Pharmacist (RPh) in the State of New Mexico.
  • Licensed Pharmacist Clinician (PhC) by the New Mexico Board of Pharmacy.
  • Registered as a Pharmacist Clinician with the Board of Medicine.
  • Certification in Basic Life Support for Healthcare Providers (BLS).
  • Advanced training/certification in anticoagulation and/or disease management.
  • Direct patient care experience as Pharmacist Clinician with prescriptive authority.
  • Experience current in disease management.
  • Fluency in Spanish preferred.
