About The Position

The Clinical Pharmacist at Monument Health is responsible for delivering comprehensive medication management in collaboration with healthcare providers to optimize patient outcomes. This role involves coordinating care across various settings and ensuring compliance with hospital standards and pharmacy regulations.

Requirements

  • Bachelor's degree in Pharmacy.
  • Pharmacist certification from the South Dakota Board of Pharmacy.
  • Advanced Cardiac Life Support (ACLS) certification from the American Heart Association (AHA).
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) within 60 days of hire or transfer.

Nice To Haves

  • Doctorate in Pharmacy.
  • Post-graduate Year 1 (PGY-1) certification from the American College of Clinical Pharmacy (ACCP).
  • Pediatric Advanced Life Support (PALS) certification from the American Heart Association (AHA).
  • 1+ years of pharmaceutical experience.
  • 3+ years of hospital experience.
  • 3+ years of clinical experience.

Responsibilities

  • Identify medication-related problems such as appropriateness of drug, dose, route, frequency, and duration.
  • Monitor patient response to therapy.
  • Provide accurate drug information to multidisciplinary team members and educational information to patients and families.
  • Manage pharmacy protocols including heparin, warfarin, pharmacokinetics, IV to PO, and therapeutic interchange.
  • Participate in quality improvement projects.
  • Lead development, implementation, and maintenance of pharmacy services in a specialty area under guidance from pharmacy leadership.
  • Act as a resource in area of specialty.
  • Document consults, pharmacist interventions, adverse drug reactions, and medication variances consistently.
