Requirements

  • Current California Registered Pharmacist license.
  • Pharm. D. degree.
  • Inpatient: Completed PGY-1 residency or at least two years clinical hospital pharmacist experience.
  • Outpatient: Completed PGY-1 residency or at least two years clinical pharmacist experience.
  • Preferred: Three years experience.

Nice To Haves

  • Completion of a pharmacy residency program in Emergency Medicine.
  • Completion of a pharmacy residency program in Infectious Diseases.
  • Completion of a pharmacy residency program in home infusion pharmacy.
  • Completion of a pharmacy residency program in Critical Care.
  • Completion of a pharmacy residency program in Ambulatory Care or General Practice.

Responsibilities

  • Provide pharmaceutical care by performing medication-related distributive, clinical, educational functions.
  • Oversee discretionary tasks performed by pharmacy technicians.
  • Review orders for appropriateness and dispense medications accurately and efficiently.
  • Consult with healthcare providers to optimize therapy and maximize safety for medication administration.
  • Guide, direct, and assist in training pharmacy technicians, new pharmacist personnel, interns, and/or residents.
  • Participate in the transition of care process of targeted patient populations.
  • Monitor and assess patient's medication and nutritional therapies.
  • Participate in interprofessional patient care rounds.
  • Provide consultation and actively participate in collaborative management of referred patients.
  • Document clinical interventions and measure the effectiveness of services provided.
