Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Past/Current Trainee Graduates Note: must send proof such as an SF-50 with application Videos Help Duties The Clinical Pharmacy Practitioner (CPP), Primary Care, utilizes expertise in the area of Primary Care to provide medication management to optimize patient care by to providing the following duties: * Attends and participates in team meetings, conferences, and other meeting as required for the purpose of making recommendations and providing consultation in all Pharmacy-related matters to Primary Care for the Pharmacy service line. * Directly assists other providers in rational medication selection by providing written and/or verbal consultation. * Is responsible for planning, developing, and maintaining effective clinical pharmacy programs by implementing rational cost-effective drug therapy to improve patient outcomes within medicine and specialty services. * Provides medication information to medicine and specialty team members as required. * Provides assistance in development of policies as requested. * Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization. * Assists in initiation of medication use improvement strategies including development of quick order sets, clinical reminders, patient/provider education handouts, and development and implementation of pharmacotherapy protocols. * Functions as an advanced practice provider to CMM services to achieve defined outcomes through direct interactions with patients and providers in assigned care area(s). * Interfaces with providers, including physicians, PAs, ARNPs in design of the veteran pharmacotherapy regimens to achieve optimization of rational and cost-effective drug therapy. * Serves as an authoritative information source on drugs and their utilization in therapy. * Promotes education to monitor, assesses and documents therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters. * Provides highly specialized education and training support to other professional services in the broad area of Primary Care medication management. * Makes recommendations to the responsible physician for optimal management of drug-induced problems either verbally or in writing when requested or initiates changes as specified under a scope of practice. * Collaborates with other TVHS staff, educating the Veterans and caregivers regarding the proper use of medications. * Serves as a pharmacotherapy expert and mentor to medical students, residents, and physician extenders, teaching them pharmacotherapy and healthcare economics. * Prepares and presents educational lectures to a variety of disciplines and patients as required. * Participates in design and/or completion of research studies and/or quality improvement projects involving drug therapy, clinical pharmacy functions, or improvements in patient care and presents it either at national meetings or publishes it in professionally representative publications. * Serves as preceptor of PGY-1 and PGY-2 residents' research projects assigned. * Responsible for coordination and supervision of all pharmacotherapy management activities in Primary Care as assigned. This included management of clinical practice within areas of expertise including scheduling, reviewing consults, finishing encounters to meet timeliness standards, providing patient care, and assuring coverage of clinical services during one's absence. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. 5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Doctor of Pharmacy degree plus (1),(2), or (3), below: 1) Completions of ASHP accredited PGY-1 Pharmacy Practice residency and Three (3) years of practice with substantial component of patient care activities acting as a mid-level provider with emphasis in Women's Health. 2) Completion BOTH an ASHP accredited PGY-1 Pharmacy Practice and a PGY-2 Ambulatory Care Residency with focus in chronic disease state management and three additional years of practice with a substantial component of patient care activities serving as mid-level provider with emphasis in Women's Health. 3) Completion of BOTH an ASHP accredited PGY-1 Pharmacy Practice Residency and a ASHP PGY-2 Specialty Ambulatory Care Residency with substantial component of patient care activities during the residency, serving as a mid-level provider with emphasis in Women's Health.
