About The Position

The Pharmacist, Clinical Specialist Oncology (PCSO) is responsible for: providing quality patient care in relation to the patient’s oncologic/hematologic diagnosis, prescribed treatment, age group, and other identified needs; providing comprehensive pharmaceutical care to oncology/Hematology patients to assure safe and effective drug therapy.

Requirements

  • Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE)
  • Minimum of 3-years’ experience with at least 50% in oncology; PGY1 residency plus two (2) years of practice experience including 50% or more of that time spent in oncology or completion of a specialty (PGY2) residency in oncology pharmacy plus one (1) additional year of practice with at least 50% of time spent in oncology pharmacy activities.
  • Current, active license to practice pharmacy in New York State; or eligible for reciprocate licensure from another state.
  • Pharmacist Board Certification Oncology, required within two years of hire or required within 2 years of eligibility to sit for certification exam.
  • Knowledge of current pharmacy and complex oncology/hematology principles and practices
  • Knowledge of computer order entry
  • Knowledge of federal and state laws governing pharmacy practices
  • Knowledge of regulatory requirements and standard of care for pharmacy, oncology and hematology
  • Knowledge of nursing and outpatient clinic procedures as they relate to pharmaceutical services
  • Ability to work independently within established policies and procedures and able to manage complex pharmaceutical patient care needs
  • Ability to accomplish established program goals and objectives
  • Ability to exercise judgment, tact, and diplomacy

Responsibilities

  • Interprets, reviews and evaluates inpatient and outpatient oncology/hematology or other chemotherapy related patient medication orders for optimal dose, dosage form, frequency, concentration, duration, drug-drug interaction, drug-disease state interaction, drug allergy interaction, drug-nutrient interactions, and potential cross allergenicity, taking into account the individuals patient’s special or age-related needs to ensure optimal drug therapy, high quality patient care and timely dispensing of pharmaceuticals. Performs pharmacokinetics dosing and monitoring for all patients on drugs where therapeutics levels are reported. Participates in multidisciplinary rounds in the inpatient and/or clinic areas with direct patient care contact for visual assessment of physical and psychological signs of drug-related effects.
  • PCSO utilizes all Pharmaceutical Standards for Sterile Compounding. Assures timely compounding and dispensing of patient specific medications assuring the right drug, route, dosage form, strength size, and quantity are selected and prepared in the appropriate concentrations to deliver the prescribed dose.  Ensures compliance with regulatory bodies regulating hazardous medications and compounded sterile preparations (CSPs).
  • Develops and maintains clinical practice in oncology/hematology, cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provides optimal selection of pharmacologic agents, and design of a regimen to maximize therapeutics benefit and minimize toxicity. Provides follow-up on the clinical effects of the regimen with adjustments as necessary to achieved desired outcomes. Develop, review, assist and maintain, in collaboration with members of the Division of Oncology/Hematology various protocols and regimens within the electronic system.
  • Participates in committees including, but not limited to:  Oncology Program Leaders, Oncology Pharmacy Committee, Pharmacy and Therapeutics Committee, Cancer Committee, and other committees as required
  • Provides drug and clinical information to other healthcare professionals in response to specific questions relating to new evolving issues related to the care of patients with cancer. Content expert for the development of oncology specific guidelines, policies and educational material.
  • Participates in supervision of the operations of the Cancer Treatment Center (CTC) satellite Pharmacy, including process improvements, completing performance reviews for the pharmacy technician specialist oncology, provides peer input to the Director of Pharmacy for performance reviews of PCPO, and maintaining the oncology/hematology formulary. 
  • Designs, supports and implements policies to provide effective and efficient use of resources and for ensuring that budget, quality and value targets are met in collaboration with the Service Line Administrator of Oncology and Director of Pharmacy. 
  • Coordinates and trains new employees and cross trains existing employees to provide efficient and professional excellence in pharmaceutical care in the CTC Pharmacy. Provides visual, spoken, and written clinical education and training to pharmacy students, pharmacists, clinical pharmacists, related to management of oncology for the Department of Pharmacy and other staff related to the safe provision of Oncologic/Hematologic Pharmaceutical Care. 
  • Ensures compliance with American College of Surgeons Commission on Cancer standards.  Clinical Trials Management: Evaluates requests for cancer-related clinical trials; Participates in local Investigational Review Board (IRB) meetings as voting member; assists Research staff in completion of Case Report Forms (CRFs) books in accordance with specific trial requirements; Participates in the preparation of trial budgets for clinical trials, contributes to patient records in accordance with FDA and ICH regulations/guidance; Ensures maintenance of inventory records of investigational pharmaceuticals. Ensures that the CTC Pharmacy function is conducted in accordance with Federal and State laws.
