About The Position

It's more than a career, it's a calling WI-SSM Health St. Clare Hospital - Baraboo Worker Type: PRN Job Highlights: This is a part time PRN Pharmacist position for St. Clare Hospital - Baraboo working a minimum of every third weekend on day shift only. There may be an opportunity to also pick up some day shifts through the week. Job Summary: Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns.

Requirements

• Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
• Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
• Other resuscitation certifications may be required dependent on department.
• State of Work Location: Illinois Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR)
• State of Work Location: Missouri Pharmacist - Missouri Division of Professional Registration
• State of Work Location: Oklahoma Pharmacist - Oklahoma State Board of Pharmacy
• State of Work Location: Wisconsin Pharmacist - Wisconsin Department of Safety and Professional Services

Responsibilities

• Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
• Documents medication related plans and interventions in electronic medical record.
• Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
• Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
• Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
• Precept pharmacy students and residents, as applicable.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.
