About The Position

Join SSM Health as a Pharmacist Clinical, Institutional with Emergency Department experience and play a critical role in delivering expert pharmaceutical care in high-acuity settings. Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns.

Requirements

• Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
• No experience required
• Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.

Responsibilities

• Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
• Documents medication related plans and interventions in electronic medical record.
• Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
• Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
• Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
• Precept pharmacy students and residents, as applicable.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.
