About The Position

This is a full time variable shift Pharmacist position for SSM St. Mary's Hospital in Madison, Wisconsin, working a total of 40 hours per week. Shifts are a combination of days and evenings and occasional overnight coverage, including an every third weekend rotation. A one-time sign on bonus may be available to qualified individuals with a 2-year service commitment. The role involves providing pharmaceutical care to patients, verifying medication orders, compounding and dispensing pharmaceuticals in accordance with federal, state, and local regulations and hospital policy. The Pharmacist serves as a resource to other healthcare professionals, including pharmacy students and residents, ensuring patient care meets standards of safety and effectiveness, and oversees the duties of pharmacy technicians and interns.

Requirements

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
  • No experience required
  • Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
  • Other resuscitation certifications may be required dependent on department.
  • Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR) (for Illinois)
  • Pharmacist - Missouri Division of Professional Registration (for Missouri)
  • Pharmacist - Oklahoma State Board of Pharmacy (for Oklahoma)
  • Pharmacist - Wisconsin Department of Safety and Professional Services (for Wisconsin)

Responsibilities

  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
  • Documents medication related plans and interventions in electronic medical record.
  • Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
  • Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precept pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
