About The Position

SSM Health is seeking an experienced pharmacist to join the institutional pharmacy team at St. Mary's Hospital in Madison, WI. This role involves providing pharmaceutical care, verifying medication orders, and ensuring patient safety and effectiveness in medication therapy. The position operates on a 7 on/7 off overnight schedule, offering a competitive salary and a sign-on bonus of up to $15,000.

Requirements

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy.
  • Registered Pharmacist license in Wisconsin, Illinois, Missouri, and Oklahoma.

  • Experience in a hospital pharmacy setting or completion of a pharmacy residency.

Responsibilities

  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
  • Documents medication related plans and interventions in electronic medical record.
  • Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
  • Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.
  • Supervises pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precepts pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
