About The Position

This position functions as a pharmacist in addition to fulfilling the role of Antimicrobial Pharmacist. The Antimicrobial Stewardship Program (ASP) clinical pharmacist specialist provides collaborative pharmaceutical care to hospital patients through the ASP. The major job responsibilities of this position are: pharmaceutical care to hospital patients receiving antimicrobial therapy, clinical consultations, cost effective care analysis, teaching, and pharmacy representation with designated committees. Requirements

• Graduate from an ACPE accredited College of Pharmacy.
• Licensed (or eligible) as a pharmacist in the state of practice as applicable.
• Residency or three years' experience in a hospital clinical setting.
• Advanced problem and critical thinking skills.
• Superior organizational and prioritization skills.
• Exceptional communication skills.
• Familiar with Current Practice Standards.
• Proficient in Microsoft Office.

Responsibilities

• Provide cost-effective pharmaceutical care to patients receiving select antimicrobial therapy.
• Participate in selection of patients receiving targeted antimicrobials and monitor these patients on a daily basis.
• Discuss medication order changes/clarifications with the infectious disease physician or the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes.
• Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
• Switch patients from IV to PO therapy as quickly as feasible.
• Provide pharmacokinetic consultations as required.
• Detect, monitor, document, and report adverse drug reactions and medications errors.
• Promote the use of the formulary by converting non-formulary orders to medications on formulary when practical.
• Facilitate discharge and maintenance of appropriate antimicrobial therapy.
• Anticipate discharge date and medications.
• Facilitate discharge planning and smooth transition to ambulatory or alternate site of care.
• Provide medication reconciliation of antimicrobial therapy and assure antimicrobial therapy regimen is addressed at discharge.
• Promote and demonstrate rational, cost-effective drug therapy.
• Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved.
• Develop and conduct target drug programs and drug usage evaluations (DUE) as needed and report results to the appropriate committee/s.
• Serve as member of the Antimicrobial Subcommittee.
• Provide quarterly report to Patient Care Committee outlining current antibiogram trends, formulary functionality and other antimicrobial metrics.
• Review, analyze and present new antimicrobials to the Patient Care Committee.
• Monitor the use of costly medications to assure their use is consistent with approved criteria and guidelines.
• Budget responsibilities include: Quarterly summary of financial and clinical metrics.
• Anticipate changes in drug therapy and report the potential impact it will represent to the overall hospital and drug budget.
• Meet or exceed the cost avoidance, savings or revenue goals of the program.
• Document clinical consultations and cost avoidance as part of the justification for the position/service.
• Facilitate investigational drug studies.
• Provide in-services for Infectious Disease investigational studies to the nurses and pharmacy staff.
• Anticipate the pharmacoeconomic impact of investigational drug studies.
• Provide financial forecasts for the Department of Pharmacy Services.
• Take a role in the education programs of the Department of Pharmacy as they relate to infectious disease.
• Precept pharmacy students.
• Precept and mentor pharmacy residents.
• Provide staff development support to pharmacists, nurses and physicians.
• Assist in development and analysis of an antimicrobial residency rotation.
• Provide presentations, publications, and other informative activities on infectious disease.
• Develop and participate in appropriate departmental quality improvement initiatives.
• Maintains competency in unique aspects of infectious disease drug therapy.
• Act as pharmacy liaison in matters relating to antimicrobials in various committees or with other departments.
• Perform duties of a staff pharmacist including medication order verification, therapeutic monitoring, dispensing pharmaceuticals, supervising non-pharmacist staff, medication use safety, controlled substance accountability.
