About The Position

The Pharmacist Anticoagulation Clinic position involves providing pharmaceutical care to patients requiring anticoagulation therapy, focusing on clinical monitoring and therapeutic interventions to improve patient outcomes. The pharmacist supervises daily patient flow and clinic activities, ensuring compliance with collaborative care agreements. Responsibilities include assessment, planning, implementation, and documentation of care for anticoagulation clinic patients, emphasizing respect, dignity, kindness, and empathy in all interactions.

Requirements

  • Graduate from ACPE accredited pharmacy college.
  • Current US pharmacist license in the state of employment.
  • Anti-coagulation certification completion within 6 months of employment.
  • Previous hospital experience.
  • Problem solving and critical thinking skills.
  • Good communication skills.
  • Familiarity with current practice standards (ASHP).

Responsibilities

  • Supervise daily clinic workflow and clinical practice of clinic staff (RN and support staff).
  • Systematically assess healthcare needs of patients receiving warfarin and/or other anticoagulants.
  • Perform continual assessment, outcomes identification, planning, implementation, and evaluation of care plans for anti-coagulated patients.
  • Interpret INR results and adjust warfarin dosages accordingly.
  • Ensure compliance with collaborative agreements directing the anti-coagulation therapy process.
  • Review and cosign visit progress notes of clinic nurses, pharmacy residents, and students.
  • Record notes and assessment data in the EPIC system.
  • Assess patient and family educational needs regarding anti-coagulation.
  • Develop relationships with patients and families to maintain trust and respect.
  • Utilize multi-disciplinary resources for patient assistance.
  • Supervise and mentor clinic nurses and support staff.
  • Participate in the development and evaluation of clinic policies and quality improvement initiatives.
  • Provide accurate drug information to medical and nursing staff.
  • Maintain professional competency and current knowledge of pharmacy practice.
