The Clinical Pharmacist work to support the vision of the Department of Pharmacy and Department of Cardiology which is optimizing patient outcomes through integrated delivery of quality pharmaceutical care and the mission of the institution. Activities include providing consultation for other health care providers, educating patients and other health care providers, reviewing and processing medication orders. Pharmacy Specialists are familiar with complex disease states and the medications used to treat them. They work closely with specialist medical providers to monitor and adjust medication regimens for these patients. The Ambulatory Clinical Pharmacist Specialist in Cardiology/Lipid clinic will Assist cardiologists in a team based approach to ensure guideline based treatment and attainment of therapeutic outcomes. Conducts patient insurance, benefits and financial investigation and verification by directly communicating with patients. Completes initial and renewal prior authorization forms, negotiate authorization status with insurers, track status of medication approval, and when necessary, alert stakeholders of denials or nonresponse. Oversees completion of prior authorizations, appeals on behalf of patients and providers, and oversee the dispensing of medications to patients in a timely manner. Works with patients, coordinators, pharmacy technicians, pharmacy support liaisons and other clinical teams to ensure that required clinical documentation is obtained prior to acceptance onto service. Documents encounters and pertinent information into the software systems used by Carilion. Provides counseling to patients and caregivers on disease states and medications used to treat them. Develops goals for care and actively works with patients towards meeting care goals. Works with patients and providers to optimize therapeutic outcomes. Ensure treatment timelines are being followed and assist with ordering labs and followup treatments based on Collaborative Practice Agreements. Engages with patients for follow ups via telephone or face to face visits based on patient needs and requests. Assists with the development and implementation of policies and procedures to ensure reimbursement of high-cost medications and maintenance of all accreditation standards. Facilitates staff training and competency, education and practice advancement. Oversee and coordinate work roles for Cardiology pharmacy technician. Coordinates with Pharmacy leadership to develop training experiences/competencies. Participate in Cardiology research and other performance improvement projects as needed and pertinent to pharmacy. Assist with transition of care activities for cardiology patients as appropriate or assigned.
