About The Position

This position will be responsible for providing pharmaceutical care to patients in the CR Wood Cancer Center ambulatory oncology clinic. The Pharmacist – Ambulatory Clinic Based Clinical Specialist (Oncology) will be a vital part of the multidisciplinary patient care team. In addition to direct patient care responsibilities including oral chemotherapy management, this pharmacist will provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, and precept pharmacy students. Requirements

  • Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE)
  • Minimum of 4-years’ experience with at least 50% in oncology within the past 7 years; PGY1 residency within the past 7 years plus two (2) years of practice experience including 50% or more of that time spent in oncology or completion of a specialty (PGY2) residency in oncology pharmacy within the past seven years, Preferred.
  • B.S. degree in Pharmacy OR Pharm.D required.
  • Pharmacist License (New York) required at hire or eligible.
  • Pharmacist Board Certification Oncology, required within 1 year of obtaining practice experience commensurate with eligibility for Board-certified Oncology Pharmacist exam.
  • Knowledge of current pharmacy and complex oncology/hematology principles and practices
  • Knowledge of federal and state laws governing pharmacy practices
  • Knowledge of regulatory requirements and standard of care for pharmacy, oncology and hematology
  • Knowledge of nursing and outpatient clinic procedures as they relate to pharmaceutical services
  • Ability to work independently within established policies and procedures and able to manage complex pharmaceutical patient care needs
  • Ability to accomplish established program goals and objectives
  • Ability to exercise judgment, tact, and diplomacy

Responsibilities

  • Provides oversight of the safe and appropriate use of medications, including prescribing, dispensing, administration, and monitoring.
  • Delivers clinical pharmaceutical care for cancer patients through collaboration with the healthcare team.
  • Reviews and clarifies medication order to ensure accuracy, completeness, and appropriateness
  • Documents pharmacotherapeutic recommendations and communicate effectively with healthcare professionals.
  • Educates and counsels patients and caregivers on chemotherapy, immunotherapy, and oral oncolytic treatments.
  • Provides medication therapy management and medication reconciliation services.
  • Conducts toxicity assessments and supports optimization of supportive care
  • Monitors patients adherence for high cost or specialty medications
