About The Position

Develops, implements, and maintains systems to address the pharmacy quality metrics of the Population Health Department and is driven to meet and exceed customer expectations. Holds self and team accountable for achieving the highest level of patient care, quality metrics, customer service, and employee satisfaction. Recognizes the contribution for desired results. Assists the director in selecting, developing, and maintaining a team of proficient staff.

Requirements

  • Doctorate of Pharmacy degree.
  • Valid Indiana pharmacy license or graduate of an accredited pharmacy school licensed as a pharmacy intern in Indiana.
  • PGY-1 Residency Training with rotations in Population Health or Ambulatory Care.
  • Pharmacist experience in population health in lieu of residency training.
  • PGY2 residency preferred.
  • BCPS or BCACP Certification preferred.
