Shift: First Shift (United States of America) Pharmacist, Ambulatory Care - Patient-Centered Medical Home (Primary Care) Nebraska Medicine – Grand Island, NE Join Nebraska Medicine as an Ambulatory Care Pharmacist supporting our growing Patient-Centered Medical Home (Primary Care) services in Grand Island. This role is part of a multidisciplinary care team focused on delivering patient-centered pharmaceutical care in the ambulatory setting. You will work closely with providers, nurses, and other care team members to optimize medication therapy, improve outcomes, and support chronic disease management across clinic populations. This is an excellent opportunity for a pharmacist who enjoys direct patient care, collaborative practice, and helping shape and grow ambulatory pharmacy services in a community-based setting. Role Highlights Provide comprehensive medication management and direct patient care in a Patient-Centered Medical Home (Primary Care) clinic setting Collaborate with physicians and care teams to optimize medication therapy and improve clinical outcomes Educate patients on medication use, adherence, and disease state management Support development of clinical protocols, collaborative practice agreements, and quality initiatives Serve as a resource for medication-related questions and provide drug information to the care team Precept and support the development of pharmacy learners and other staff as appropriate Shift Full Time, 1.0 FTE, 40 hours per week First Shift Located in Grand Island, Nebraska Why Work at Nebraska Medicine? Lead provision of patient-oriented pharmaceutical care services including creation and maintenance of pharmaceutical care plan, cost-effective therapeutic drug monitoring, drug utilization, guideline development, and drug use evaluation in assigned clinical teams. Provide training and educational support for development of clinical skills in professional staff. Responsible for dissemination of new knowledge, utilization trends and outcomes information related to medication use/pharmaceutical care programs at the organization. Lead efforts to educate other disciplines regarding appropriate use of drug therapy.
