About The Position

This position is designed for individuals who are passionate about improving patient experiences and outcomes. The Pharmacist will work collaboratively within a clinical care team, engaging with prescribers and other healthcare professionals to ensure that patients understand their health care plans, medication changes, and adherence to prescribed therapies. By fostering trust and building meaningful relationships with patients, the Pharmacist plays a crucial role in making healthcare more accessible and effective. The Pharmacist, Ambulatory Care will participate in daily huddles with clinicians and medical assistants, reviewing patient medications, testing results, and identifying care gaps. This collaborative approach allows the Pharmacist to make informed recommendations that enhance patient satisfaction and lower medical costs. Additionally, the Pharmacist will support patients in managing chronic conditions under a collaborative practice agreement, ensuring that medication reconciliation is coordinated across various care settings. As part of CVS Health's commitment to continuous improvement, the Pharmacist will provide feedback on the clinical care model, helping to differentiate CVS from its competitors. This role also includes mentoring other pharmacists and residents on best practices within the clinical care model, contributing to the overall development of the pharmacy team. The Pharmacist may also assist with other CVS Pharmacy initiatives as capacity allows, demonstrating flexibility and a willingness to support the organization’s broader goals.
