About The Position
PHMC is proud to be a leader in public health. The Family Navigator provides support, education, and resources to PHLpreK families and Early Learning Centers to connect with Pre-School Early Intervention, behavioral health, and services. This role is essential in helping families navigate the complexities of obtaining necessary services for their children. The Family Navigator connects families to needed supports by addressing known barriers to obtaining services, including monitoring the screening process and supporting childcare center staff in effective family engagement. This position provides direct support to families throughout the intake process and problem-solves barriers to obtaining services. The Family Navigator partners with Early Intervention staff and Behavioral Health providers to provide training and educational materials to teachers and parents. They support families and PHLpreK staff in making referrals for additional services where necessary and appropriate. This role requires a proactive approach to ensure families are informed and supported in accessing the resources they need. The Family Navigator will also engage in ongoing professional development to support continuous quality improvement and knowledge of family engagement and support practices. This position reports to the Provider Service Coordinator and involves working closely with families, educators, and community resources to ensure that children receive the necessary early intervention services. The Family Navigator plays a critical role in enhancing the overall well-being of children and families within the PHLpreK program.
