About The Position

The Clinical Support Provider Personal Health Navigator (CSP-PHN) at WVP Health Authority is a Traditional Healthcare Worker who acts as a liaison between clients, healthcare providers, and community resource organizations. This role focuses on supporting equitable and quality care for clients by coordinating referrals, documenting interactions in the EHR system, and engaging with the community to promote health navigation services.

Requirements

• Must have THW certification and be able to apply for Personal Health Navigator through OHA.
• Ability to pass a background check for being on public school premises.
• Current Oregon Driver's License and a good driving record.
• Bilingual in English and Spanish.
• Ability to effectively interact with co-workers and clients from diverse backgrounds.
• Computer and word processing experience.

Responsibilities

• Providing direct resource management to clients.
• Updating case load lists for the supervisor weekly.
• Reviewing new incoming referrals and communicating with referral sources for updates and coordination.
• Documenting client interactions within the EHR system in a timely manner.
• Building and maintaining relationships with treatment and community resource organizations.
• Maintaining documentation, status reports, statistical data, and other required documents.
• Presenting in the community about Healthcare Navigation Hub services.
• Performing other duties as assigned.
