Requirements

  • Ability to read, analyze, and interpret general business periodicals and professional journals.

  • Ability to solve practical problems and deal with concrete variables.

Responsibilities

  • Report all changes in the resident’s condition to the Nurse as soon as practical.
  • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
  • Report all accidents and incidents observed during the shift.
  • Agree not to disclose resident’s protected health information and report suspected violations.
  • Report any unauthorized attempts to access the facility’s information system.
  • Perform only care procedures that have been trained for.
  • Ensure the resident’s room is ready for receiving the resident.
  • Greet residents and escort them to their room.
  • Introduce residents to their roommate and other personnel as appropriate.
  • Make residents comfortable and inventory their personal possessions.
  • Assist residents with packing personal possessions during transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles as necessary.
  • Create and maintain a warm and calm environment throughout the unit.
  • Meet regularly with nursing personnel to identify and correct problem areas.
  • Report all complaints and grievances made by residents.
  • Participate in and receive nursing reports as instructed.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds as instructed and put extra covers on beds as requested.
  • Check residents who are unable to call for help frequently.
  • Answer resident calls promptly and check personal care needs.
  • Assist residents with food arrangements and after meal care.
  • Perform assigned tasks in accordance with established policies and procedures.
  • Cooperate with inter-departmental personnel to maintain services.
  • Notify the facility when late or absent from work.
  • Report occupational exposures to blood, body fluids, and hazardous chemicals.
  • Assist residents with daily functions such as dental care, bathing, and dressing.
  • Keep residents dry and change soiled clothing and linens.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Check for pressure areas and skin breakdowns to prevent bedsores.
  • Report injuries of unknown sources and observe disoriented residents.
  • Record and report data as instructed and perform special treatments.
  • Notify the Nurse of any resident leaving or missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Maintain confidentiality of resident care information.
  • Report incidents of unauthorized disclosure of information.
  • Report allegations of resident abuse or misappropriation of property.
  • Honor resident’s refusal of treatment requests and report to supervisor.
  • Assist residents with personal care functions including hair and nail care.
  • Shave male residents and keep female residents' hair clean.
  • Assist residents with bowel and bladder functions.
  • Maintain intake and output records as instructed.
  • Provide eye and ear care as instructed.
  • Weigh and measure residents as instructed.
  • Check each resident routinely to ensure personal care needs are met.
  • Assist with post mortem care as instructed.
