Requirements

You must be able to communicate effectively (verbally and in writing) with scheduling coordinators regarding the member's health while maintaining confidentiality per HIPAA regulations.
Be able to communicate with various types of members and use resources to communicate with members that are hard of hearing, linguistically impaired, and/or emotionally distraught.
This position requires extensive communication either telephonically, electronically or in person.
This position will require standing, walking, and lifting between 10-40 lbs.

Nice To Haves

experience as a sitter/companion/homemaker is a plus

Responsibilities

Provides supportive household services by performing light housekeeping duties, laundry, grocery shopping, etc. in accordance with the written plan of care.
Prepares nutritious meals including medically prescribed diets.
Follows service plans goals tasks.
Documents observations and activities.
Recognizes and reports any changes in client’s physical, mental and social status to supervisor and agency staff involved in the client’s care.
Performs care and services with minimal or no client/family complaints.
Assist with personal care for advantage members as per directed by service plan goals
PCA will assist with advanced supportive restorative (ASR) task with proper training from LPN or RN as specified in members per service plan and goals.
Participates in agency activities.
Attends agency staff meetings.
Attends a minimum of 12 hours of in-services per year.
Performs care and hours of services as outlined on care plan.
Must be willing to travel between members and run errands
