About The Position

The Personal Care Assistant supports the mission, vision, and values of Golden Valley Health Centers Central Valley PACE Center by performing patient care activities and other tasks as assigned to assist the nursing staff. THIS IS A HOME CARE POSITION. Position will work at our Central Valley PACE Center in Modesto, CA. Scheduled Monday – Friday, working from 8:00am – 5:00pm. Compensation: $22.19 - $23.29 an hour Duties and Responsibilities Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants. Communicates patient care needs to the LVN/Supervising RN and update Manager. May assist in Day Center and Activities when there is down time. Deliver DME to PRT’s home and document in EMR. Flexible to work weekends when needed. Home Care Assessments. Communicates effectively with participants, families, staff, other departments and agencies. Works effectively in a team setting. Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms. Participates in teaching/learning process with others. Follows guest relations policies and procedures. Abides by all established policies, rules and regulations. Attends staff meetings and takes part in participant care planning. Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse. Communicates effectively with participants, families, staff, other departments and agencies. Works effectively in a team setting. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties. Performs patient care activities appropriately, reporting any circumstances or change in patients condition. Other projects and duties as assigned. Physical Demands Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff. Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents. Must have high manual dexterity. Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling. Work Environment Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment. Subject to unpleasant odors. The noise level is usually quiet to moderate, but may at times be noisy and crowded.

Requirements

  • Valid California Driver’s License, acceptable driving record and vehicle insurance.
  • Must have reliable transportation.
  • Must be able to treat people of all ages including geriatrics.
  • Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.
  • High School Diploma or equivalent required.
  • Current BLS CPR Card certified by the American Heart Association
  • Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.
