About The Position

Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home. Schedule: PRN (as needed) Position Summary: The Personal Care Aide (PCA) provides personal care and supportive services to PACE participants in collaboration with the Interdisciplinary Team (IDT) and in accordance with the plan of care (POC). Services are primarily delivered to the participants’ homes but may also be provided in the PACE center or other care settings, including long-term care or assisted living facilities. The PCA supports participants’ safety, comfort, and independence by assisting with activities of daily living, maintaining a clean and safe environment, and observing and reporting changes in participant condition to clinical staff.

Requirements

  • High school diploma or General Education Degree (GED) required. Must be at least 18 years of age.
  • Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
  • Must maintain CPR certification.
  • Knowledge of safety and infection control requirements for healthcare facilities.
  • Knowledge of medical equipment use and maintenance.
  • Demonstrated ability to provide compassionate care and effectively communicate with the adult and elderly population.
  • Must be a licensed driver

Nice To Haves

  • One year of home health aide experience preferred.
  • Use of technology-based tools, including electronic medical records, preferred.
