About The Position

JOB SUM M ARY CNAs and PCAs p r ovide n onmedical se r vice and assis t ance to a client in their h o me and co m munity, who, becaus e of advan c ed age o r physical or m ental disa b ility, cannot p erform these serv i ces. These services ena b le the clien t to remain in hi s /her reside n ce. Therefore, the caregi v er must fee l comfortab l e caring for a clien t in a priv a te home or s e tting. The Ca r egivers mai n objective i s client satis f action and safety for bot h oneself an d for the client. Th e Caregi v er must h a ve sensitivit y for the nee d s of others , a professional manner, and a positive attitude . Aides ar e responsible fo r ensuring t h at service is del i vered in a caring and res p ectful manner, in accordance wit h th e Agen c y polices, in d ustry standar d s, and state regulations. Aide s needs t o report o n time to sched u led shi f t unless specific circumstan c es come up. ESSENTIAL F U NCTI O NS The CN A /PCA p r ovides se r vice to a client ba s ed on a n d limited to a client’s care plan. The care plan is based on an assessme n t of the client’s need s fo r services, deve l oped i n conjunctio n with a fun c tional assessment a n d with the i n put from the client an d /or client’s family. C N A/PCA service s are tasks that a r e necessitated by a client’s d i sability or chronic conditio n that limits the client’s ability to perfo r m funct i onal activities or daily living (A D L’s) and instr u mental activ i ties of dai l y living (IADL’ s ). CNA/PCA serv i ces vary dependin g on the needs a n d requirements of each in d ividual client. Client ne e ds and requirements are documente d through th e client’s funct i onal asse s sment. These serv i ces involve direct care fr o m cueing/promptin g to total assistance. Aides will p r ovide excelle n t client c a re includin g , but not l i mited to, ass i stance wi t h the followin g : Ambulation Range of Mot i on Bathing Personal Hyg i ene Dressing Grooming Feeding Exercise Transferring Positioning Toileting and I n continence Car e Medication R e minders Housekeeping Meal Plannin g and Prepar a tion Laundry Transportation Shoppin g and Errands Companionship Vital Signs an d Blood Pres s ure CNAs and PCAs will: Ensure client’s sa f ety and s ecurity by supe r vising the h ome envi r onment Teach/perform M e al Planni n g and Preparat i on and s e rving the client Routine Hous e keeping a ctivities suc h as maki n g/cha n ging bed d ing, dustin g , vacuumin g , washing floors, cleani n g kitchen a n d bathroom, an d laundry. H ousehold tasks shoul d not exc e ed 20% of t i me worked pe r shift unless th e client has a Hou s ekeepin g Aut h orization. Provide Compa n ionship in c ludin g social interactions, c onversations, e motional reass u rance, and encourageme n t of activiti e s that simula t e the mind.   • Provide Companionship including social interactions, conversations, emotional reassurance, and encouragement of activities that simulate the mind
  • Provide respite care for families in accordance with care plans
  • Preform/assist with essential shopping/errands , which may include handling the client’s money in accordance with the care plan, agency policy, and under the observation of the Supervisor or Administrator
  • Assist clients with following written, special diet plan, and reinforcement of diet maintenance, which is provided under the direction of a Physician and as identified on the care plan
  • Assist clients with standing, walking, sitting, and rolling in bed
  • Escort clients to medical facilities, errands, shopping, and outings as specified in the care plan
  • Assist clients with communication by writing or typing correspondence for them or researching information for them
  • Participate on the Care Team by providing input and making suggestions
  • Notifies agency when unable to work scheduled assignments – 24 hour notification required
  • Ensure service is delivered in accordance with all relevant policies, procedures, and practices
  • Monitor supplies and resources
  • Follow the written care plan and works all hours as established by Supervisor
  • Carry out duties as assigned by the Supervisor
  • Observe clients and their environments and immediately reports unsafe conditions to Supervisor
  • Observe clients and their environments and immediately reports behavior, physical and/or cognitive changes and / or changes in living arrangements to Supervisor
  • Complete and maintain records of daily activities, observations, and direct hours of service. Submits written documentation to the office, as required
  • Attend orientation, in-service training sessions, and staff meetings
  • Develop and maintain constructive and cooperative working relationships with others
  • Make decisions and solve problems
  • Communicate regularly with Supervisor and co-workers
  • Observe, receive, and obtain information from relevant sources
  • Report any incidents or accidents immediately to Supervisor

  • Maintain client’s protected health information confidential and observes all client’s rights
  • Report, detect, and prevent abuse and neglect
  • Aides will perform other duties as required
