Requirements

  • High school diploma or GED
  • Minimum one (1) year of documented home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred
  • Certified CPR and negative TB skin test or chest x-ray
  • Must have a valid driver's license and auto insurance (proof of insurance is required)
  • Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client.
  • Must be able to lift up to 50 pounds
  • Self-directing with the ability to work with little direct supervision
  • Ability to adhere to HIPAA and ensure confidentiality and security of the client's medical information
  • Ability to read, write, speak, and understand English and communicate effectively
  • Up-to-date vaccinations including COVID-19, Influenza, and Hepatitis B
  • Ability to travel within Northeast Ohio territory, up to 60 miles from Middleburg Heights in some cases
  • Must have flexible availability Monday through Friday and some weekends

  • Preferred: Successful completion of a nursing assistant or certified home health aide program

Responsibilities

  • Assist with personal care tasks (bathing, grooming, toileting, dressing)
  • Willing to travel within 35 minutes across the Westside
  • Provide companionship and emotional support
  • Light housekeeping and meal preparation
  • Monitor and report changes in client condition
  • Follow care plans and communicate with case managers
  • Provide client-facing care as assigned
  • Provide assistance with daily tasks as directed in the client's personalized plan of care, including but not limited to: feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shopping
  • Provide companionship, personal and hygiene care as directed in the client's personalized plan of care, including but not limited to: bathing, grooming, oral care, dressing, undressing, toileting activities, and assisting with measurements, if necessary
  • Assist clients with active and passive mobility work defined by the plan of care, and document observed changes to the Director of Nursing
  • Assist with various office / administrative tasks
  • Participate in office and field on-call as needed
  • Assist the scheduling team with assessing the competency of staff to promote quality, continuity, and safety of care for the agency's clients
  • Train and educate field staff as needed to ensure services are provided in accordance with brand standards
  • Complete new caregiver introductions for home care cases and staffing clients
  • Participate in the intake of new clients and onboarding of new employees
  • Assist office staff with coordinating and executing employee and client appreciation activities throughout the year
  • Assist the office and staff with other duties as needed
  • Adhere to the policies, procedures, and protocols of the agency of employment
  • Adhere to HIPAA laws and ensure confidentiality and security of the client's medical information
