About The Position

Permanency Coordinator Philadelphia Regional Office EDUCATION: Bachelor’s Degree in Social Work or a related field required. Master’s degree preferred. EXPERIENCE: Experience working directly with children and families in the areas of foster care and adoption. SUPERVISORY REQUIREMENTS: None STATUS: Full-time; Non-exempt QUALIFICATIONS: Must have excellent organizational, verbal and written communication, and inter-personal skills. Must demonstrate leadership tenants and the ability to work independently with children and families. Knowledge of Kinship Foster Care services and adoption preferred. Must possess excellent case management and client assessment skills. Must have the ability to work effectively and cooperatively with other professionals and service providers.

  • Valid Pennsylvania Driver’s License
  • Registered and insured vehicle
  • Certificate of Insurance for vehicle
  • Valid PA Inspection (even if vehicle is registered in another state)
  • Valid/Clean Motor Vehicle Record
  • Current Act 33, Police Clearance and FBI Clearance
  • Available to work flexible schedule (weekend hours as scheduled)
  • City/County travel
  • On Call

Responsibilities

  • Child and Family Profile Responsibilities Write comprehensive child and family profiles and remain current on all SWAN profile expectations
  • Complete the child profile within 90 days of accepting the referral
  • Complete the family profile within 120 days of accepting the referral
  • Update/complete the benchmarks on the SWAN portal on a monthly basis
  • Child Preparation and Adoption Finalization Responsibilities Meet the expectations of completing adoption finalizations and child prep as needed
  • Complete home visits and meet with families monthly
  • Train families on the adoption and PLC process
  • Permanency Preparation Completion of CSR, Family Matching, and Placements units of service as requested
  • Keep track of and update permanency data weekly
  • Assist with facilitating unit meetings
  • Facilitate permanency meeting with CUA agencies and providers
  • Review completed work for quality and accuracy
  • Additional Work on-call hours as directed
  • Interface with all service providers for the purpose of service delivery and goal planning meetings
  • Prepare monthly progress reports, service plans and other reports are required by DHS/CUA, DPW, and the agency
  • Adhere to all other program and agency policies and procedures governing the objective of the agency and program
  • Performs other related duties as assigned
