About The Position

The Perinatal Community Health Worker (pCHW) serves as an integral member of the Care Transitions Management team, contributing specialize knowledge of hospital systems, community resources, and perinatal support services. The pCHW is responsible for advancing equitable maternal health outcomes by supporting marginalized pregnant individuals and their infants through culturally responsive education, advocacy, and care coordination. This role is designed to address maternal health inequities by equipping birthing individuals with the information, guidance, and resources necessary to effectively manage their health and well-being. To ensure the delivery of high-quality and informed support, the pCHW participates in specialized training that prepared them to meet the complex medical, social, and emotional needs of expectant parents.

Requirements

  • High School Diploma or Equivalent Required
  • Completion of 120-160 Hrs Community Health Worker Core Training Required
  • Completion of Lifestyle Coach Training Required
  • Completion of Perinatal Training Required
  • 3 years as a Certified Community Health Worker with active employment providing services in care transitions Required
  • General Experience supporting maternal and infant health Required
  • Cert Community Health Worker Required

Nice To Haves

  • Associate's Degree Preferred
  • General Experience n medical or health related field and knowledge of medical terminology Preferred
  • General Experience placing patients into community based programs and/or knowledge of community based resources Preferred
