About The Position
Per Diem Social Worker - Hospice Albany The Community Hospice has a per diem opportunity for a social worker to join our hospice care team caring for patients in the Capital region as part of our Interdisciplinary Team in homes and Nursing homes. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is Per diem Monday- Friday 8a-4:30p. Days are flexible.
Requirements
- NYS SW Licensure.
- Valid NYS Drivers license.
- Minimum 2-3 years experience ideally caring for seniors or those with advance illness.
Responsibilities
- Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care.
- Provide specialized skills and mental health therapy for clients.
- Identify barriers which impact optimal patient functioning.
- Ensure patients receive appropriate resources in most appropriate setting.
