Requirements

  • Kaweah Health approved crisis intervention training within 60 days of hire/transfer.
  • Associate's degree or equivalent experience in a medical setting working with an interdisciplinary health care team.
  • Medical terminology experience.
  • Excellent communication, interpersonal, and writing skills.
  • Excellent organizational and problem solving skills.
  • Able to work independently as well as within the team.

Responsibilities

  • Assists in financial counseling; linking patients and their families to appropriate resources in collaboration with the Social Worker and RN Case Manager.
  • Participates in Interdisciplinary Team meetings and assists in discharge planning as needed, or as directed by the Social Worker or RN Case Manager.
  • Facilitates charity assistance for needed medication or clinical visits for qualified Residents as authorized by PFS Manager or Director of Case Management.
  • Responds to Advance Health Care Directive orders and follows up with patient and families to ensure compliance with Joint Commission requirements.
  • Assesses and coordinates transportation via taxi and/or bus to facilitate timely discharge.
  • Assesses and coordinates assistance with obtainment of essential medications for the indigent patient population.
  • Coordinates with Case Management, Social Workers and Financial Counselors to ensure that patients discharge needs are being met.
